Regardless of age
Metastatic prostate cancer
Very high risk: T3bT4 or primary Gleason pattern 5 or >4 cores with grade group 4 or 5
High risk: T3a or grade group 4 or 5 or PSA >20 ng/ml
It appears to recommend genetic testing, and Russo et al extend this testing to germline testing,
Any other risk group with any of the following
Ashkenazi Jewish ancestry
Prostate cancer family history: brother, father, or multiple family members diagnosed with prostate cancer at <60 years old or died from prostate cancer (grade groups 25)
3 cancers on the same side of the family (especially if diagnosed at 50 years old): bile duct, breast, colorectal, endometrial, gastric, kidney, melanoma, ovarian, pancreatic, prostate (grade groups 25), small bowel, or urothelial
